Registration Requriements

BCSD Registration Requirements

The following documents are required to complete the registration process:

  1. Two Proofs of Residency: (Proofs must contain name, address, and recent date)

    Please provide ONE item from Category 1 AND one from Category 2. If an item from Category 1 is unavailable, please provide at least TWO from Category 2.

    Category 1

    • Mortgage Statement
    • School or Property Tax Receipt
    • Lease Agreement
    • Homeowner's/Renter’s insurance policy
    • A statement by a third-party landlord, owner, or tenant from whom you lease or with whom you share property within the district
    • Third-party Affidavit that establishes your physical presence in the District


    Category 2

    • Pay stub
    • Income Tax Form
    • Utility or other bills
      (Utility bills preferred: electric, water, cable, landline telephone, gas)
    • Voter Registration document
    • Membership documents (e.g., library cards) based upon residency and including address and recent date on the card
    • Official driver’s license, learner’s permit, non-driver identification, vehicle insurance
    • State or other government-issued identification
    • Documents issued by federal, state, or local agencies (e.g., local social service agency, federal Office of Refugee Resettlement)
    • Evidence of custody of the child, including but not limited to judicial custody orders or guardianship papers|

       
  2. Documentation of Age
    a) If available, a certified transcript of a birth certificate or record of baptism (if this is available, no other documentation regarding child’s age may be used)
    b) Where the documentation listed in (a) is not available, a passport
    c) Where the documentation listed in (a) and (b) is not available, the District can consider other documentary or recorded evidence in that has been in existence for at least two years. Such other evidence may include, but is not limited to:

    • Official driver’s license
    • State or other government-issued identification
    • School photo identification with date of birth
    • Consulate identification card
    • Hospital/health records
    • Military dependent ID Card
    • Documents issued by federal, state, or local agencies (e.g., local social services agency, federal Office of Refugee Resettlement)
    • Court orders or other Court-issued documents
    • Native American tribal documents
    • Records from non-profit international aid agencies and voluntary agencies
       
  3. Child’s immunization records and most recent physical from their doctor
